What to send: 1) Completed form below or written story (150 words or less will be included for free; bios over the first 150 words will be charged 15¢/word). 2) Up to two photos, one from your early days in the fraternity, plus a current photo. (Additional photos may be included for $15 per extra photo). NOTE: All materials will be returned to you after publication. You don’t have to order a book to be included. If you do not wish to write your biography, simply fill out the form below and an editor will compose it so you will be included in this historic book at no charge.
